Importante po na kayo dapat ang designated beneficiary sa insurance policy ng Member-Insured at ACTIVE and hindi LAPSED ito at the time of death upang maka-claim sa AFPMBAI. Salamat po.

1. Death Certificate issued by Local Civil Registrar
2. Unit Casualty Report
3. Report of Death from Adjutant General and Major Services (PGAB) (for Philippine Army only)
4. Statement of Service/Service Record
5. Latest Payslip
6. Spot/Investigation Report
7. CENOMAR of both husband and wife (original copy); CENOMAR of deceased member, if single
8. Certificate of Legal Beneficiaries (from the Unit)
9. Certificate of Legal Guardianship of minor beneficiaries (for minor beneficiaries only)
10. Photocopy of Dependent's ID card
11. Marriage Contract of Parents (if deceased member is single)
12. Birth Certificate of any sibling-beneficiaries (marriage contract, if already married)
