Your death benefit proceeds shall be given to your designated beneficiaries indicated in your Permanent/Optional Insurance Contract and/or MBAI Protek Basic Membership Insurance Certificate. In the absence of such, if you are single, the proceeds shall be given to your parents.

Dapat po palaging UPDATED ang inyong membership information with AFPMBAI, lalo na if you change your civil status.

Mag-fill out po ng Membership Information Form (MIF) to update your membership info and designated beneficiaries for your MBAI Protek basic insurance plan.

For your permanent/optional insurance plan, please fill out an Amendment Form. 

Kung kayo naman ay new entrant to the service, mangyari po lamang na mag-fill out ng MBAI Protek application form sa inyong Personnel Office or Human Resource Management Center para ma-create kaagad ang inyong account with AFPMBAI at para maka-avail kaagad ng ating loans at financial assistance.
