TO ALL RETIRED/RETIRING AFPMBAI MEMBERS:

Please file for your Termination Benefit para makuha po ninyo ang inyong MEMBER'S EQUITY FUND na galing sa 'MBAI PROTEK' o 'AFP MBA P' na deduction ninyo sa payslip.

Ayon sa batas (Insurance Code, R.A. 10607), kailangan pong ibalik ng AFPMBAI sa inyo ang kalahati (50%) ng lahat ng inyong basic membership insurance monthly contributions, plus interest.

Nasa 25 to 60 pesos lang po ang inyong monthly contributions hanggang 2009 (under Old Basic Insurance). Tumaas po ito sa halagang 0.5% (for AFP) o 1.5% (for DILG- and DOTR-attached agencies) noong 2010 hanggang sa kasalukuyan kung saan uniform 1.5% of base pay na po ang monthly contribution (under the new basic insurance).

REQUIREMENTS FOR TERMINATION BENEFIT CLAIM:

1. Retirement order / Discharge Order/Separation Order

2. Updated Service Record / Statement of Service Certificate of last payment

3. Latest payslip

4. Photocopy of Valid ID (back to back)

5. Original copy of policy certificate/ contract (may request at any AFPMBAI Office)

6. Living Benefit Form (available at our offices or download at bit.ly/AFPMBAIForms

7. Notarized Quit Claim form
