2. Updated Service Record / Statement of Service Certificate of last payment and Latest payslip
3. Photocopy of Valid ID (back to back)
4. Original copy of policy certificate/ contract (may request at any AFPMBAI Office)
5. Living Benefit Form (available at our offices or download at bit.ly/AFPMBAIForms
6. Notarized Quit Claim form
