If you cannot visit the nearest branch office, you can apply online by EMAIL.

Step 1 – Download niyo po ang Loan Application Form sa https://afpmbai.com.ph/afpmbai-forms/ and print on clean long sized bond papers. Pls FILL OUT, SIGN and affix your THUMBMARKS on the AUTHORITY TO DEDUCT AND AUTHORITY TO DEPOSIT as well.

Step 2 - Ipapa-approve po ninyo ito sa inyong CO at Finance at kailangan na rin po ng NOTARYO for all loans paid thru payroll deduction. (Hindi na po kailangan kung direct payment sa Policy Loan.)

Step 3 - Pls take CLEAR MOBILE PHOTOS of the signed loan form, unexpired Service ID (w/ 3 specimen signatures), ATM (front only, pakisulat po ang ATM number at cellphone number), and selfie pic with your Service ID & ATM.
