Please SIGN-UP for MBAI iProtek (Plan 499 for Retired Enlisted Personnel or Plan 999 for Retired Officers), download the application form at bit.ly/AFPMBAIForms and submit the signed forms to the nearest AFPMBAI branch or extension office.
Please pay the first monthly contribution over-the-counter or thru our online payment facilities (GCASH, Maya, Land Bank Mobile Pay, and Metrobank Online) and other direct payment partners (SM Bills, Palawan Express, LBC & Cebuana Lhuillier).
For those who are about to retire from the service, you may avail of the offer to continue your membership insurance with AFPMBAI by deducting the MBAI iProtek premiums from your Termination Benefit or Equity Value AND later on from your monthly retirement pension, WITHOUT the need for insurability requirements (medical exam).
To avail of this privilege, retiring members must submit the following application forms at the nearest AFPMBAI Office within thirty (30) days from date of retirement/discharge:
a. Authority to Deduct from Termination Benefit for MBAI iProtek for Retiring Members, and
b. Authority for Pension Deduction for MBAI iProtek of Retiree Members -- to commence on the starting date of pension

Nagsubmit ako ng Cash Surrender Form in your Bacolod Branch last 08 August 2023. Bumalik ako today 07 Sept 2023 to have an update sa transaction ko. Sadly after 1 month since nasubmit ko ang form wala pa din nangyari. Andun pa din sa opisina nila. Ang dahilan naka leave ang in charge na personnel. Grabe naman po yan. 1 month na wala pa din nangyari. I also did an email, text and call sa mga numbers ng office ninyo peri wala din sumasagot. Nakakadismaya lang po na pinapahirapan ninyo ang inyong member. At kung alam niyo lang kung gaanu kalayo ang unit assignment namin sa opisina ninyo para personal na mag update kasi hindi kami masagot sa tawag, text at email.
